District 7750 Projects Start Here : Instructions & Club Codes To access the Online application form, click here: www.scdmvonline.com/DMVNew/forms/MV-95.doc. Complete the form and, in the block for type of special plate requested, type “Palmetto Rotary License Plate” then add your “Club Code xxxx” in the same block. Print the completed form (You need only the first page of the 3 page form.). Or print the blank form and complete it by hand. Now you can mail the completed form and check to DMV per their instructions and ours. Or take the form and your checkbook to any DMV office and walk out with your Rotary tag. History & Background In 2003, Rotary Districts 7750 and 7770 formed a joint License
Plate Committee to gather the 400 applications needed to create a special
Rotary “vanity plate”. That was accomplished and the plates can be ordered
from any Department of Motor Vehicles office. We believe this project serves
two very worthy causes in our Districts. First, it puts the Emblem of Rotary
International on license plates therefore giving it exposure. We feel that if
you, as a Rotarian, have this plate, someone will see it, ask you about it and
give you the opportunity to share what Rotary means to you. Second, it is a
great way to raise needed funds for The Rotary Foundation and for District
projects that help people right here in South Carolina.

A higher registration fee is required if your vehicle is classified as a truck, but the $50 charge is constant.
If you are a District Governor or Assistant District Governor, you may be able to get a two digit number. Application for these numbers must go through the District (contact below) and not directly to DMV. If you have any questions, please contact:
